🩺 Group Health Insurance for Medical & Dental Practices
Physician-owners, associate dentists, and clinical staff all raise the same question slightly differently — here’s how a practice’s group plan actually gets structured.

🩺 Does a medical or dental practice face different group health rules than other small businesses? Why It’s Different
The ACA mandate and California small-group rules apply the same way to a medical or dental practice as to any other business — there’s no special carve-out. What’s different in practice is the mix of people on your payroll: a solo or small partnership of clinicians, a hygienist or associate who may work reduced hours, and front-office/clinical support staff, each of whom can raise its own classification question for your group plan.
🤝 An Extension of Your Business
👨⚕️ “Think of us as an extension of your business. We carry the admin load — quotes, census, enrollment — so your team stays on what it does best.”
👩⚕️ “Most owners never count how many hours open enrollment takes. Behind me is one of California’s top General Agencies and its Professional Suite of quoting, census and enrollment tools. That’s real admin time back on your calendar.”
👨⚕️ “More than an agent. A partner in your growth, your technology and your service, every renewal.”
👩⚕️ “So how many hours did your team spend on benefits paperwork last year?”
🩺 Do physician-owners or partners count toward my FTE total?Owner Status
It depends on the legal structure. Sole proprietors, partners in a partnership, and more-than-2%-owners of an S-corp generally aren’t counted as common-law employees for ACA full-time-equivalent purposes, and they’re also excluded from the Small Business Health Care Tax Credit calculation. A physician who’s a W-2 employee of a professional corporation, by contrast, typically is counted like any other employee. If your practice has multiple owner-physicians under different structures, it’s worth confirming each one’s status individually rather than assuming they’re all treated the same way.Source: IRS — Small Business Health Care Tax Credit; IRS Employer Shared Responsibility Provisions
💵 Should I expect higher premiums because of my industry?Cost
Not inherently. California’s small-group market uses community rating, meaning your premium is based on your employees’ age, location, and family size — not your industry or claims history. A medical practice doesn’t pay more just for being a medical practice. Where industry does matter is plan design: many practices offer an HSA-qualified option alongside a traditional plan, since clinical staff often skew toward routine, predictable care needs.
